Secure Your EMS License Plate Today!
We are happy to report that the legislation has passed both the General Assembly and the Governor has signed the bill into law.  We can now move forward with processing the license plate and setting up our EMS Memorial Scholarship and Training Council.  This specialty plate provides us with an opportunity for us to show our pride for our occupation as well as to assist our colleagues and their families through funds which will be disseminated through the EMS Training Council.

The goal of these specialty plates is to raise funds to provide scholarships and assistance to family members of EMS personnel killed in the course of their employment.  EMS personnel risk their lives every day on the job to save others, however, unfortunate incidents do occur which often put our lives at risk.  Through the EMS Memorial Scholarship Council, we hope to lessen the burden faced by the family members who are left behind due to unforeseen, tragic events.

How can you secure a license plate?

The Secretary of State will begin production of the EMS license plate only after receiving 1500 requests for the plate.  Requests are available to the general public and license plate numbers are given out on a first come first served basis.  An applicant needs to fill out the request form (below) to hold a spot and enclose $12.00 deposit made payable to the Illinois Secretary of State.  Once 1500 requests are in place, the Secretary of State will begin production.

Once license plate production begins, an applicant is charged an additional $27.00 fee for original issuance of their specialty plate (your $12.00 deposit will be applied to this $27.00 fee).  Of the $27.00 fee, $15.00 is deposited into the Secretary of State’s Special License Plate Fund and $12.00 is deposited into the Illinois EMS Memorial Scholarship and Training Fund.  Each year when you renew your plates an additional $17.00 fee is charged.  Of that fee $2.00 is deposited into the Secretary of State Fund and $15.00 is deposited into the EMS Memorial Scholarship and Training Fund.

If you are interested in preserving your spot, please complete the enclosed form and return the form and a check made payable to the Illinois Secretary of State in the amount of $12.00 in the enclosed envelope.
